We are pcs'ing to Germany, and when I spoke with EFMP she told me I needed some form to take to my sons school because he just started special ed classes. I could barely understand the lady. She said the form number, but I couldn't catch it all. Do any of you know where I get this form from? 

Id just go to the office and pick up the paperwork, they'll know what you need. 

Just go to the EFMP office at Madigan and get a packet. You'll need the forms filled out by the school, probably also a copy of his IEP, and an EFMP exam by his doctor.
